Institutional Representatives should survey their institution of all participants. Checks should not be sent by individual participants. Your Institutional Representative should send funds to cover your expenses.
On the Institutional Registration Form: for "Faculty Attending" entry:
Please list the name(s) of each individual Faculty member(s) attending,
and enter "Banquet" after each name of the Faculty member(s) that will be attending the Banquet.
On the Institutional Registration Form: for "Students Attending" entry:
Please list the name(s) of each individual student(s) attending,
and enter "Banquet" after each name of the student(s) that will be attending the Banquet.

The cost is $25 per person for the conference and an additional $40 for the banquet.
